Dayton High School

The following douments will be needed at registration:

• Student's birth certificate
• Student's social security card
• Student's immunization record
• Proof of residency - Utility bill in parent/guardian name with physical address. 
• Example: electric bill, water bill, telephone bill, or cable bill
• Provide one bill in your name with the physical address on it. The date on the bill cannot be more than 30 days old. We annot accept disconnect notices.  If you are unable to provide a bill in your name, please speak to the counselor's secretary.
• Driver's license of parent/guardian enrolling the student
• Withdrawal from any school attended from the prior school year

Incoming 9th graders must have proof of promotion as well as the following immunizations:

1. (1) Tdap or Td within the past 10 years
2. (1) Meningitis Vaccine (MCV4)
3. (2) Varicella Vaccines (or have had chicken pox)

• Unofficial transcript or course history showing courses taken and grades received from sending school for 10th, 11th, and 12th grade.
• Copy of last ARD or 504 plan
• Must provide Dayton ISD Power of Attorney papers if not living with parent(s).
